초록

소아치과에서 협조가 되지 않는 어린이의 치과치료 시 사용하는 진정법은 환자안전을 최우선으로 고려하여야 하며, 의료사고예방을 위한 evidence-based 진료가 필요하다. 근거중심의학의 부상에 따라 치의학계에도 근거중심치의학(Evidence-Based Dentistry)에 대한 관심이 점차 증가하고 있으나 한국의 진정법에 대한 근거중심의 체계적인 연구는 거의 이루어지지 않고 있다. 이 연구의 목적은 체계적인 문헌고찰법을 이용하여 지금까지 국내에 보고된 소아치과진정(요)법을 검토하고, 향후 국내의 근거중심 소아치과 진정법의 발전 방향을 모색하는 것이다.

본 연구는“한국보건의료연구원의 체계적인 문헌고찰 지침”을 참고하여 국내 5개 데이터베이스 1) Core 검색 데이터베이스- KM base, KISS 2) 학술정보 및 포털 -국회도서관, DBpia, RISS를 검색하였다. “치과진정법”및“치과진정요법”으로 검색어를 제안한 총 470편의 논문 중 문헌선택과정의 흐름도를 따라 중복제거 하고 남은 31개의 논문 중 원문이 제공되는 문헌을 모두 취합한 총 20개 문헌을 분석대상으로 삼았다.

이번 체계적 문헌고찰은 한국 소아치과 영역에서 지난 25년간 시행된 치과진정법에 관한 일관된 근거(evidence)를 확인하기 위하여 GRADE 과정에 정의된 객관적인 기준에 기초하여 진행된 결과, 전체적인 근거는 보통(moderate)의 질로 평가되었다. 그러나, 특히 약물의 추가 투여 부분에 대해서는 매우 낮은(very low)의 질로 평가되었다. 그러므로 향후 안전한 약물의 사용 부분에 대해 보다 체계적으로 잘 고안된 임상 실험이 필요하다.

Abstract

The first priority of sedation for incorporative children in pediatric dentistry is a safety. Therefore, evidence-based practices in health care are needed for preventing medical accidents. In accordance with the rise of the evidence based medicine, the interest in Evidence-Based Dentistry is increasing in the field of dentistry.

However, systematic research about Evidence-Based sedation in Korea has rarely been done. As such, the purpose of this systematic review is to critically analyze the available scientific literature regarding dental sedation and to seek the next developmental strategies about evidence based pediatric dental sedation.

A broad search of the 5 databases of the systematic reviews manual of the National Evidence-based Healthcare Collaborating Agency in Korea were referenced: 1) Core search database- KMbase, KISS; 2) Academic information and portal; 3) the National Assembly Library; 4) DBpia, and 5) RISS. Of a total 470 themes limited to the search term of“ dental sedation”, in accordance with the PRISMA statement for reporting systematic reviews of health sciences interventions, a literature selection process, which includes the removal of overlapping down the flow chart, was performed. Of the remaining 31 articles, two authors read through articles independently and added or removed articles using the exclusion criteria.

Finally, twenty published papers of acceptable quality were identified and reviewed. This systemic review of Korean pediatric dental sedation practices for the last twenty-five years was based on the objective criteria defined in the GRADE process and identified consistent evidence. The results were evidence of moderate quality. Therefore, more systemically well-designed clinical studies are needed about the safe use of a sedative medicines (drugs).

Ⅰ. 서 론

치과 진료를 위해 치과에 방문하는 어린이들은 본능적인 공포를 가진다. 그런 공포를 경감해 주기 위해 진정법은 유용한 수단이 될 수 있으나 그에 따른 대비가 반드시 필요하다[1]. Klingberg 등[2]은 호주, 캐나다, 유럽, 미국의 어린이 중 9%가 치과 진료 시 공포, 불안감, 행동 관리 문제를 경험한다고 보고하였다. 치과에 대한 공포(Dental fear, DF)란 치과에서 경험하는 특정 자극에 대한 무서움을 뜻하며, 불안감(Dental anxiety, DA)이란 진료에 대한 막연한 두려움으로 이 두 가지 용어는 흔히 혼용되며, 치과에 대한 부정적인 느낌을 공포와 불안감(Dental fear and anxiety, DFA)으로 표현하였다. 그리고 치과 진료 시의 비협조적인 행동으로 인한 행동 관리 문제(Dental behavior management problems, DBMP)는 치료를 지연시키거나 치료를 불가능하게 하기도 한다고 하였다[2]. 치과 의사는 진료 중 어린이의 행동 관리에서 어려움을 겪는 경우가 많으며 주로 치과에서 받는 스트레스가 비협조적인 행동의 원인으로 생각된다[3]. 심한 치과적 공포와 불안으로 치과적 행동 조절에 문제를 일으키는 환자의 경우, 진정법과 같은 약물적 행동 조절 방법이 요구될 수 있다[4].

진정법이 국내에 소개되고 본격적으로 시행된 지도 30여년이 지났고, 2004년 전문의제도가 시행된 후로 10년이 넘는 시간이 흘렀다. 더욱이 2014년부터는 전문의 표방이 가능하도록 제도가 바뀌었다[1]. 최근 대한소아치과학회 산하 진정법위원회에서 진정법을 사용하는 소아치과 회원 215명에게 2014년 2월 메일로 설문을 보내어 응답한 111명의 자료를 분석한 결과 지난 20년 동안 진정법의 시행이 많아지고 있었다. 그러나‘치과 진정법 진료지침’에 대해 78%가 인식하고 있으나 활용정도에 대해서는 꽤 참조하는 편이라는 응답이 17% 밖에 되지 않아 진료지침의 활용정도는 매우 부족한 상황이다[5].

한편 근대 이후 과학이 발달하면서 의학도 과학적 근거에 의해 발달해 왔다. 그러나 임상진료가 지금까지 항상 과학적 근거에 의해 이루어졌다고 보기는 어렵다. 의료인이 대학을 졸업하고 국가시험에 합격하여 면허를 취득한 후에는 실제로 충분한 보수교육이 이루어지지 않는다. 의학의 발달이 느리게 진행되던 과거에는 과거에 습득한 지식과 계속 쌓여가는 경험에 의지하여 환자를 진료하는 것이 가능하였다. 그러나 지금은 지식이 폭발적으로 증가하고 있으며 교과서는 최신의 정보원이 될 수 없고 수많은 저널들에 발표되는 기사들을 개인이 모두 읽어보고 비교 평가하여 정확한 정보를 얻기는 매우 힘든 실정이다. 최근에 인터넷을 비롯한 정보기술의 발달은 방대한 의학정보의 수집과 체계적 검토를 가능하게 하였다. 이러한 변화와 필요성에 따라 최근에 부상한 새로운 개념이 근거중심의학(Evidence-Based Medicine)이다[6].

근거중심의학은 보건의료분야의 의사결정을 과학적인 근거에 기초하여 하는 것으로서, 의학이 갖추어야 할 조건이고 의료인이 지녀야 하는 지식과 기술이다. 나아가 보건정책 등 의료분야 전반에서 그 중요성이 인식되고 있다. 근거중심의학이라는 용어를 처음 사용한 것은 1992년에 캐나다 McMaster 대학의 Guyatt 연구진이며[7], 근거중심의학 관련 문헌 수는 1992년에 한 개의 문헌에서 시작하여 1998년에 약 1,000개로 증가하였고 지금은 기하급수적으로 증가하고 있다. 근거중심의학의 실행에 있어 핵심적인 내용인 체계적 고찰(systemic review)은 우리나라에서도 활동이 늘고는 있으나 아직 활성화되기에는 많은 과제가 놓여있다. 체계적 고찰은 명시적인 방법을 통해 과학적인 근거를 통합하고 제시하여 신뢰할 수 있는 객관적 근거를 제공하는 근거중심의학의 핵심적인 영역의 하나이다. 특정한 주제에 대하여 문헌을 객관적으로 평가하고 엄격한 방법론을 사용하여 종합된 추정치 등을 제시하는 것이다[8].

소아치과에서 협조가 되지 않는 어린이의 치과치료 시 사용하는 진정법은 환자안전을 최우선으로 고려하여야 하며, 이를 위해 evidence-based 진료가 필요하다[5]. 근거중심의학의 부상에 따라 치의학계에도 근거중심치의학(Evidence-Based Dentistry)에 대한 관심이 점차 증가하고 있으나 아직 이에 대한 국내의 관심과 발전은 더딘 실정이다. 국내 소아치과학 분야에서도 대한소아치과학회의 학술대회나 대한소아치과학회지 기사에서 근거중심 소아치과학(Evidence-Based Pediatric Dentistry)에 관한 발표나 논의가 희소하였으며[6], 한국의 치과 진정법에 대한 근거중심의 체계적인 연구는 거의 이루어지지 않고 있다. 이 연구의 목적은 체계적인 문헌고찰법을 이용하여 지금까지 국내에 보고된 소아치과 진정(요)법을 검토하고, 향후 국내의 근거중심 소아치과 진정법의 발전 방향을 모색하는 한편 근거중심의 치과진정법 진료지침 개발의 기초 자료로 활용하기 위한 것이다.

Ⅱ. 연구 대상 및 방법

본 연구는 “한국보건의료연구원의 체계적인 문헌고찰 지침[9]”을 참고하여 국내 5개 데이터베이스 1) Core 검색 데이터베이스- KM base, KISS 2) 학술정보 및 포털 -국회도서관, DBpia, RISS를 검색하였다. “치과진정법” 및 “치과진정요법”으로 검색어를 제안한 총 470 편의 논문 중 문헌선택과정의 흐름도를 따라 중복제거 하고 남은 263개의 논문 중 회색문헌 및 배제기준을 제외하고 원문이 제공되는 문헌을 모두 취합한 총 20개 문헌을 분석대상으로 삼았다.

1. 연구 대상

본 연구의 분석대상은 국내 주요 문헌 데이터베이스 5개를 이용하여 “치과진정법”을 주제로 한 논문 470편이다. 관련문헌 검색은 국내 데이터베이스 1) Core 검색 데이터베이스- KM base(한국의학 논문 데이터베이스, http://kmbase.medric.or.kr), KISS(학술데이터베이스 검색, http://kiss.kstudy.com), 2) 학술정보 및 포털 - 국회도서관(http://www.nanet.go.kr), DBpia(http://www.dbpia.co.kr), RISS(학술연구정보서비스, http://www.riss4u.net/index.jsp)를 기반으로 검색하였다. 검색 시기는 각 데이터베이스의 자료제공일 및 저널 발행일로부터 2015년 8월까지 발간된 논문을 대상으로 하였다. 검색어는 국문검색어 “치과진정법” 및 “치과진정요법”으로 제한하였다. 조사 대상의 선택과 배제 요인은 Table 1과 같다.

2. 연구 방법

국내 문헌 데이터베이스 5개에서 “치과진정(요)법”이라는 검색어로 논문을 검색한 후, 분석문헌을 선택하기 위해 원문이 제공되는 문헌을 모두 취합하여 각 데이터베이스 별로 자료를 받아 엑셀로 저장하였다. 검색된 “치과진정(요)법”과 관련이 없는 논문, 검색어가 논문제목에 포함되어 있으나 원문이 제공되지 않는 학술대회 초록집, 치과진정법이란 단어가 제목에 각각 분리되어 의미가 달라지거나 모호해진 논문, 저자요청 등의 이유로 서지정보만 제공되고 원문은 제공되지 않는 논문, 회색문헌(석,박사학위논문, 연구보고서, 포스터 및 구연자료, 편집인의 글)을 배제하였다(Fig. 1).

1차적으로 검색된 문헌은 KMbase 4편/0편, KISS 20편/0편, 국회도서관 24편, DBpia 0편/0편, RISS 252편/180편으로 총 470편이었다. 이 중 중복여부를 확인하고 논문의 제목과 초록을 수기로 확인 후 자료의 원문 제공여부 확인을 거쳐 남은 문헌 수는 86편이었으며, 위의 배제 기준을 적용하여 선정한 총 20편의 논문을 최종 연구 자료로 선택하였는데 이는 Table 2와 같다.

3. 문헌의 질 평가

GRADE(Grading of Recommendations Assessment Development and Evaluation) System(GRADE working group, 2004, http://www.gradeworkinggroup.org) [10]을 이용하여 문헌의 질을 평가하였다. 문헌의 질은 높음, 보통, 낮음, 매우 낮음으로 분류되었다.

Ⅲ. 연구 성적

주 연구 대상 논문들의 질 평가 결과 무작위 대조군 연구(RCTs)와 코호트(COHORT) 연구는 한 편도 존재하지 않았다(Table 3, 4).

1. 사용된 약물과 투여 경로

연구에 사용된 약물들을 살펴보면, Chloral hydrate (20편 중 15편, 75%), Hydroxyzine (20편 중 14편 70%), Midazolam (20편 중 11편, 55%)이 가장 많았고 N₂O-O₂와 Meperidine 등이 있었다. 투여 경로는 경구와 직장 내(Chloral hydrate), 비강 내, 점막 하, 근육 내, 정맥 내(Midazolam) 등이었다.

2. 사용된 약물의 용량과 연령 및 표본크기

1) Chloral hydrate과 Hydroxyzine

박과 김[11]은 구강 내로 Chloral hydrate 50 mg/kg 단독, Chloral hydrate 50 mg/kg과 Hydroxyzine 25 mg 병용 및 Chloral hydrate 50 mg/kg을 직장 내 투여를 비교한 결과 55%, 75%, 35%의 성공률(Lunt의 방법[31])을 보고했다.

정과 박[15]은 Chloral hydrate 60 mg/kg와 Hydroxyzine 25 mg을 투여했을 때 69.6%성공률(Houpt의 방법[32]을 변형함)을 보였고 42.2%에서 일시적 저산소증을 보였다고 하였다.

이, 김과 권[16]은 Chloral hydrate 70 mg/kg 단독과 추가로 Hydroxyzine을 kg 당 1 mg, 2 mg, 3 mg을 투여시 Automated counting system (ACS)를 이용하여 객관적 행동양상 평가(OSUBRS, ohio state university behavior rating scale)와 주관적 치료효과 평가 결과를 조합하여 Chloral hydrate 70 mg/kg와 Hydroxyzine 2 mg/kg이 효과적이라고 하였다.

홍과 이[18]는 Chloral hydrate 50 mg/kg와 Hydroxyzine 25 mg만을 투여했을 때 63.3%가, Chloral hydrate 50 mg/kg와 Hydroxyzine 25 mg에 30% N₂O와 70% O₂를 혼합사용 시 93.3%에서 만족할 만한 진정효과를 얻을 수 있다고 하였다.(평가방법은 Houpt의 방법[32]) N₂O-O₂를 병용한 경우에서 Chloral hydrate와 Hydroxyzine 성공률이 높은 이유는 1971년 Hogue 등[33]의 주장과 같이 첫째, N₂O-O₂의 사용이 경구 투여된 진정제의 약효를 높이는데 기여하였으며, 둘째, N₂O-O₂의 적용이 구강점막의 감각이 둔화되어 개구기의 장착 또는 마취 시의 자입 자극 등에 대한 동통의 역치가 높아진 것으로 판단된다고 하였다.

노 등[22]은 Chloral hydrate 50 mg/kg과 Chloral hydrate 70 mg/kg을 Hydroxyzine 2 mg/kg와 50% N₂O-O₂를 혼합 사용 시 고용량에서 더 효과가 높았다고 하였다.

최, 정과 양[17]은 Chloral hydrate 75 mg/kg와 Hydroxyzine 25 mg만을 투여했을 때와, Chloral hydrate 75 mg/kg와 Hydroxyzine 25 mg에 30% N₂O와 70% O₂를 혼합사용 시 Midazolam 0.1 mg/kg 근육주사(IM)와 30% N₂O와 70% O₂를 혼합사용 시 Chloral hydrate 75 mg/kg와 Hydroxyzine 25 mg에 Midazolan 0.1 mg/kg 근육주사(IM)와 30% N₂O와 70% O₂를 모두 사용 시 (Houpt의 방법[32]) 비교한 결과 Chloral hydrate와 Hydroxyzine을 복합 투여한 진정방법이 Midazolam(근주 IM)과 N₂O-O₂를 혼합사용보다 더 효과적인 진정효과를 얻을 수 있었다고 하였다.

2) Midazolam

김과 이[3]는 Midazolam 0.2 mg/kg을 비강 내 투여 시 69%의 성공률(Houpt의 방법[32])을 나타냈는데 36개월 미만보다 이상의 연령에서 더 높은(75%) 성공률을 보였다고 하였다.

박 등[19]은 Midazolam 0.2 mg/kg을 비강 내 투여 시와 Midazolam 0.5 mg/kg을 구강 내 투여 시 (Houpt의 방법[32])효과를 비교한 결과 경구투여는 경비투여와 유사한 적절한 진정효과를 가지면서 환자에게 더 잘 받아들여지는 투여방법으로 생각된다고 하였다.

김, 김과 유[21]는 Midazolam 0.3 mg/kg(최대 5 mg)을 근육 내 투여와 N₂O-O₂(30-50%) 후 Midazolam 0.1 mg/kg과 0.2 mg/kg을 정맥 내 투여 시 효과를 비교한 결과, 용량이 증가되었을 때 임상적 성공률(Automated counting system(ACS)를 이용하여 객관적 행동양상 평가(OSUBRS, ohio state university behavior rating scale)와 주관적 치료효과 평가 결과를 조합)이 높았으나 통계적으로 유의하지는 않았다고 하였다.

3) 기타

김, 김과 박[20]은 Chloral hydrate 60 mg/kg와 Hydroxyzine 25 mg과 Midazolam 1 mg/kg과 Meperidine 1 mg/kg을 (Houpt의 방법[32]) 같은 어린이에서 한 달 간격으로 시행 후 효과를 비교한 결과 Chloral hydrate와 Hydroxyzine을 경구 투여한 경우가 Midazolam과 Meperidine을 경구 투여한 경우보다 우수한 진정효과를 보인다고 하였다.

이 등[24]은 Chloral hydrate 60 mg/kg, Hydroxyzine 1 mg/kg과 N₂O-O₂(50%) 군과 위 세 가지에 추가로 Midazolam 0.1 mg/kg 점막하 주사를 병용한 군의 비교 결과 점막하 주사를 이용한 Midazolam의 추가 투여는 안전성을 크게 침해하지 않는 범위 내에서 진정 깊이를 깊게 한다고 하였다.

김, 정과 백[16]은 Chloral hydrate 50 mg/kg, Hydroxyzine 1 mg/kg과 N₂O-O₂(50%)에 추가로 Midazolam 0.2 mg/kg을 비강 내 투여와 점막하 주사 투여한 군의 비교 결과 두 가지 경로에서 차이가 없다고 하였다.

Ⅳ. 고 찰

이번 국내 체계적 문헌고찰 결과, 치과진정(요)법에 사용하고 있는 여러 약물의 용량과 병용 및 투여방법에 따른 성공률의 차이와 효과에 대한 다양한 비교 연구들이 진행된 것으로 조사되었다. 치과진정법시 주로 사용하고 있는 Chloral hydrate(CH), Hydorxyzine(Hy), Midazolam(Mdz), N₂O-O₂의 용량과 투여경로는 미국소아치과학회 및 대한소아치과학회의 기준을 따르고 있었다. 미국소아치과학회에서 실시한 조사에 의하면, 미국소아치과의사들의 70-80%가 경구투여에 의한 진정요법을, 약 85%는 N₂O-O₂에 의한 흡입진정법을 사용하고 있는 것으로 나타났다[34]. 국내 연구에서는 1999년 최와 심 등[35]에 의하면 흡입, 경구, 근육 내, 비강 내, 정맥 내, 직장 내 순으로 약물을 투여한다고 하였으며, 2004년 안 등[36]에 의하면 경구, 흡입, 비강 내, 근육 내, 정맥 내, 직장 내 순으로 약물을 투여하며 Chloral hydrate(CH), Hydorxyzine(Hy)과 N₂O-O₂를 병행하여 사용하고 있는 경우가 60%, Chloral hydrate(CH), Hydorxyzine(Hy)만 사용하는 경우가 34%라고 하였다. 2014년 진행된 설문조사 결과, Chloral hydrate(CH) 50-70 mg/kg, Hydorxyzine(Hy) 1-2 mg/kg, Midazolam 근주(Mdz, IM) 0.1-0.2 mg/kg가 한국 소아치과의사들이 선호하는 치과진정법 약물의 용량이며, 치과진정법 시 선호하는 약물의 조합은 CH+Hy+N₂O-O₂(67.6%), CH+Hy+Mdz 점막 하 주사(SM)+N₂O-O₂(29.7%), Mdz IM+ N₂O-O₂(23.4%) 등으로 3가지 이상의 약제를 함께 사용하고 있었다. 48%는 약제의 추가투여를 하고 있으며 주로 Midazolam을 사용하여 점막 하 주사를 하고 있었다[5]. 그러나, 진정법의 부작용은 3가지 이상의 약물을 조합하여 사용하는 경우와 관련이 깊다[37,38]. 2014년 국내 조사 결과, 치과진정법시 부작용을 경험한 응답자는 87.5%로[5], 10년 전 진행된 안 등[36]의 조사결과(41%)와 비교 시 2배 이상의 증가를 보였다. 부작용은 주로 오심과 구토, 과 흥분, 호흡저하 및 호흡곤란, 비틀거림 등으로 이전 연구들과 유사하였다[39]. 그러므로 최근 조사결과를 비춰볼 때, 국내 치과진정법시 2가지 이상의 약제들을 병용하는 경우의 증가와 그에 따른 부작용 발생이 증가되고 있으나, 사용되는 약제들의 안전한 용량에 대한 정확한 진료지침이 없어 이에 대한 연구 및 진료지침의 제정이 필요하다.

Ⅴ. 결 론

이번 체계적 문헌고찰은 한국 소아치과 영역에서 지난 25년 간 시행된 치과진정법에 관한 일관된 근거(evidence)를 확인하기 위하여 GRADE 과정에 정의된 객관적인 기준에 기초하여 진행된 결과, 전체적인 근거는 보통(moderate)의 질로 평가되었다. 그러나 특히 약물의 추가 투여 부분에 대해서는 매우 낮은(very low)의 질로 평가되었다. 그러므로 향후 약물의 추가 투여 부분에 대해 보다 체계적으로 잘 고안된 임상 실험이 필요하다.
