Abstract

전신적 또는 국소적 요소에 의하여 유치 치근이 병적으로 흡수될 수 있으며 치아 우식증이나 외상 등으로 염증성 치근 흡수가 발생할 수 있다. 유치의 병적 치근 흡수면은 생리적인 치근 흡수와 비교할 때 흡수면의 세포와 흡수와의 형태 등의 흡수 양상이 다르다.

이 연구는 생리적 치근 흡수, 외상, 염증성 치근 흡수면의 형태 및 인접한 세포를 주사전자현미경과 광학현미경으로 관찰하였다. 생리적 치근 흡수면의 흡수소와는 원형과 타원형이고 비교적 작고 균일하였다. 외상으로 인한 치근 흡수면과 염증성 치근 흡수면의 흡수소와는 생리적 치근 흡수면의 흡수소와에 비해 다각형이며 크고 불규칙하였다. 생리적 치근 흡수면과 외상으로 인한 치근 흡수면, 염증성 치근 흡수면에서 다핵거대세포와 단핵세포가 관찰되었으며 염증성 치근 흡수면에서는 간엽세포 및 염증세포가 많이 분포되어 있었다. 생리적 치근 흡수면과 외상으로 인한 치근 흡수면에서는 흡수 표면에 백악질양 조직이 침착되었으나, 염증성 치근 흡수면에서는 관찰되지 않았다.

Trans Abstract

Local and general factors have been attributed to root resorption occurred by injuries such as trauma and dental caries that affect periodontal ligament or dental pulp tissue. Pathologic root resorption is different from physiologic root resorption in terms of resorption pattern such as micromorphology of resorption fossae and types of observed cells.

Microscopic morphologies and histologic features of physiologic and pathologic root resorption surface of maxillary primary central incisors resulting from trauma and periapical inflammation were observed by scanning electron microscope and light microscope. The morphology of physiologic resorption lacunae was small and oval or circular shape with regularities. The morphology of pathologic resorption lacunae was large and polygonal shape with irregularities compared with the physiologic resorption lacunae. Multinucleated giant cells and mononuclear cells were closely attached to the physiologic and pathologic resorption lacunae, whereas several kinds of mesenchymal cells with numerous inflammatory cells were found in the areas adjacent to the pathologic resorption surface. Compensating cementum formation took place along some of the areas of physiologic and pathologic resorption area resulting from trauma, but could not be observed on pathologic resorption area resulting from periapical inflammation.

Ⅰ. 서 론

유치의 치근 흡수는 전신적 혹은 국소적 요인에 의해 일어날 수 있으며 생리적 치근 흡수와 병적 치근 흡수로 분류할 수 있다. 유치의 생리적 치근 흡수는 계승 영구치의 맹출에 의한 압력이 주요한 자극 인자로 작용하며 지속적인 과정이 아닌 흡수와 재생이 반복되는 과정이다[1-3]. 생리적인 치근 흡수는 파치세포와 대식세포, 섬유아세포, 백악아세포 및 호중구 등의 다양한 간엽 세포를 포함하고 있는 육아조직에 의하여 진행되며[4], 파치 세포가 치아의 경조직 흡수에서 주된 역할을 한다[3,5].

병적 치근 흡수는 정상적인 영구치 맹출과 관계 없이 외상이나 치근단 염증, 종양, 낭, 과도한 교합력, 매복치, 재식술 등에 의해 발생하는 과정이며[6], 치수 괴사나 염증, 석회화, 흡수와 침착 등으로 인하여 유치의 병적 치근 흡수가 일어날 수 있다고 알려져 있다[7].

치근 흡수면의 형태학적인 차이를 관찰하는 것은 흡수의 진행 양상과 세포의 기능을 추정하는데 도움이 될 수 있다. 현재까지 유치의 흡수에 관여하는 다양한 세포들의 기능이 밝혀졌으며, 치근 흡수에 따른 파치세포의 분화과정에 대해 보고된 바 있다[3,8-13]. 박 등[14]은 유치의 생리적 흡수와 염증성 치근 흡수면을 비교하여 형태학적 차이를 관찰하였고 Sreeja 등[15]은 생리적으로 흡수된 유구치와 치근단 육아종, 교정 치료, 치성 종양으로 인해 병적으로 흡수된 영구치의 치근 흡수면을 주사전자현미경으로 관찰하여 보고한 바 있다[14]. 그러나 유치의 생리적 치근 흡수, 외상성 외흡수, 염증성 흡수 세 가지 유형의 흡수면의 형태학적인 차이를 비교 관찰한 연구는 없었다.

기존의 연구는 유치와 영구치를 비교하거나 상, 하악 유절치나 유구치를 모두 연구 대상에 포함하였다[12,14-17]. 여러 부위의 치아를 대상으로 할 경우 치근 흡수를 일으킨 원인 이외의 다른 요소가 영향을 미칠 수 있는데, 그 중 첫 번째 요소는 유전치와 유구치의 치근 흡수 양상이 다르다는 점이다. 유전치의 설측에 영구 치배가 위치하기 때문에 치근단의 설측으로부터 흡수가 시작되나, 구치부에서는 영구치배가 유구치의 직하방의 치근 사이에 위치하기 때문에 근원심 치근 사이의 치조골이 흡수되고 점차 분지부의 치근면이 흡수된다는 차이점이 있다. 두 번째 요소로 치아 부위에 따른 해부학적 차이에 의해 상아질 흡수면의 형태학적 차이가 있을 수 있다. 그러므로 위와 같은 요소들을 배제하기 위한 목적과 함께 상악 유중절치가 유아기 우식증이나 외상 등으로 치근 흡수 빈도가 높으므로 연구 대상을 한정하여 관찰하였다.

이 연구에서는 상악 유중절치의 생리적 흡수, 외상성 흡수, 염증성 흡수 등 세 가지 다른 원인에 따른 입체적인 표면 구조, 세포의 조성 및 조직학적 차이를 비교 연구하기 위해 치근 흡수면을 주사전자현미경과 광학현미경으로 관찰하였다.

Ⅱ. 연구 재료 및 방법

1. 연구 재료

이 연구는 연세대학교 치과병원 소아치과에 내원한 전신 병력이 없는 건강한 5 - 8세 사이의 어린이를 대상으로 생리적 및 염증성 치근 흡수를 보이는 상악 유중절치를 발거하여 사용하였고, 연세대학교 치과대학병원 윤리위원회(Institutional Review Board, IRB)의 승인 하(IRB No: 2-2011-0031)에 진행하였다. 생리적 치근 흡수 표본은 중절치의 맹출로 인하여 치근의 2/3이상이 흡수되어 중등도 이상의 동요도를 보이는 상악 유중절치를 사용하였고, 외상성 치근 흡수 표본은 외상으로 인한 외흡수로 치근의 1/2 - 2/3정도 흡수되었으나 치근단 방사선 투과성 병변이 관찰되지 않는 상악 유중절치, 염증성 치근 흡수 표본은 치근단 염증으로 치근의 1/2 - 2/3정도의 흡수 소견이 있고 방사선 사진상 치근부에 명확한 방사선 투과성 병변이 관찰되는 상악 유중절치를 사용하였다.

2. 연구 방법

1) 주사전자현미경 관찰

생리적으로 치근 흡수된 상악 유중절치 5개와 외상으로 인해 치근 흡수된 상악 유중절치 5개, 치근단 염증으로 인해 염증성 치근 흡수된 상악 유중절치 5개를 관찰하였다. 치아는 발거 직후 0.1 M phosphate buffer (pH=7.4)로 조정한 Karnovsky고정액(2% glutaraldehyde, 2% paraformaldehyde, 0.5% CaCl2)에 6시간 이상 전 고정한 후 0.1 M phosphate buffer로 2시간 수세하고 1% OsO4로 고정하였다. 후 고정한 표본은 0.1 M phosphate buffer로 10분간 수세 후 50%에서 100%까지 alcohol로 탈수하고 isoamyl acetate로 치환 후 약 30분 - 1시간 후에 임계점 건조 후 ion coater를 이용하여 30 nm 두께로 금박 처리하였다. 표본 중 육아 조직이 부착되어 있는 치아는 치근 흡수면 관찰이 어려우므로 발거 후 즉시 5% NaOCl에 2시간 처리하여 흡수면에 부착된 육아 조직을 제거하였다. 처리된 15개의 유치 치근 흡수면을 주사전자 현미경(FE SEM S-800, Hitachi, Japan)으로 관찰하였다.

2) 광학 현미경 관찰

생리적 치근 흡수된 상악 유중절치 5개와 외상으로 인한 치근 흡수된 상악 유중절치 5개, 치근단 염증으로 인해 염증성 치근 흡수된 상악 유중절치 5개를 관찰하였다. 치아는 발거 직후 10% 중성 formalin에 24시간 고정하였고, 10% ethylenediamine-tetraacetic acid (EDTA) 용액에서 탈회시켰으며 paraffin에 포매하여 치아 장축(longitudinal) 방향으로 5 ㎛ 두께의 절편을 제작하였다. Hematoxyline-eosin (H-E) 염색을 시행한 후 광학 현미경(Olympus BX40-32HO2, Japan)으로 치근 흡수양상을 관찰하였다.

Ⅲ. 연구 성적

1. 주사전자현미경 관찰

1) 생리적 치근 흡수면 관찰

상아질 흡수와 내에 접시모양의 수많은 흡수소와들로 이루어져있으며(Fig. 1A), 흡수소와는 타원형, 원형의 모양을 가지고 상아세관이 뚜렷하며(Fig. 1B), 흡수소와 내에 5 - 10개의 단핵세포들이 뭉쳐져 있었다(Fig. 1C). 표면을 덮고 있는 교원섬유와 방추형의 섬유아세포, 대식세포도 일부 관찰되었다(Fig. 1D).

2) 외상으로 인한 치근 흡수면 관찰

주로 유치의 한쪽 면에서만 흡수가 진행되는 양상으로 치근 흡수면이 비스듬하게 형성되었으며 불규칙하고 날카로운 변연을 보였다(Fig. 2A). 흡수소와의 크기는 생리적 치근 흡수면보다 다소 크고 불규칙하였으며 비스듬하게 형성된 흡수소와에서 상아세관이 관찰되었다(Fig. 2B). 흡수소와 내에 거대 파치세포가 보였으며 파치세포 주변으로 여러 개의 단핵세포가 보였고(Fig. 2C) 백악질 흡수 표면은 두터운 섬유성 결체 조직으로 덮여 있었다(Fig. 2D).

3) 치근단 염증으로 인한 염증성 치근 흡수면 관찰

흡수면의 형태는 다양하게 나타났으며 불규칙하며 직경 20 - 30 ㎛의 크고 깊게 형성된 다각형의 형태였다(Fig. 3A, 3B). 상아세관과 흡수소와 내에 단핵세포 무리가 보였다(Fig. 3C). 흡수표면은 치밀한 섬유성 결체 조직과 대식세포, 다수의 혈구세포로 덮여 있었다(Fig. 3D).

2. 광학 현미경 관찰

1) 생리적 치근 흡수면 관찰

치근첨에서 백악-법랑 경계쪽으로 흡수되는 전형적인 유치 흡수 양상을 보였으며 파도모양의 큰 흡수와와 이를 구성하는 흡수소와가 나타났고(Fig. 4A) 흡수면을 따라서 부분적으로 백악질양 조직이 침착되어 있었다(Fig. 4B). 작은 흡수소와 내에 존재하는 다핵거대세포와 단핵세포들이 관찰되었다(Fig. 4C, 4D).

2) 외상으로 인한 치근 흡수면 관찰

편측으로 치근이 흡수된 양상이 관찰되었고(Fig. 5A) 상아질 흡수면에 백악질양 조직이 부분적으로 침착되어 있었다(Fig. 5B). 불규칙한 흡수소와를 보였으며 흡수소와 내에 다핵거대세포와 단핵세포들이 존재하고 간엽세포들과 성긴 결합조직을 관찰할 수 있었다(Fig. 5C, 5D).

3) 치근단 염증으로 인한 염증성 치근 흡수면 관찰

흡수면의 형태는 다양하게 나타났으며 일정한 흡수 양상을 보이지 않았다(Fig. 6A). 불규칙한 흡수소와들이 관찰되었고(Fig. 6B), 흡수소와 내 다핵거대세포와 단핵세포를 관찰할 수 있으며 특징적으로 림프구 등 다수의 염증세포가 관찰되었다(Fig. 6C, 6D). 흡수면을 따라서 백악질 침착은 관찰되지 않았다(Fig. 6A - 6D).

Ⅳ. 총괄 및 고찰

유치의 생리적 치근 흡수 과정에서 계승 영구치의 맹출에 의한 압력이 중요한 역할을 한다. 계승 영구 치배의 맹출에 의해 성상 세망의 세포로부터 parathyroid hormone (PTH)-related protein (PTHrP) 및 interleukin 1α가 방출되어 치배의 치낭세포를 자극하여 colony stimulating factor-1, monocyte chemotactic protein-1 등과 같은 단핵구 유인 인자를 분비하며 단핵구들이 융합하여 파치세포 및 파골세포 등의 경조직 흡수 세포로 분화한다[18,19]. Receptor activator of nuclear factor kappa B (RANK) 및 RANK ligand (RANKL) 시스템이 파치세포 및 파골세포의 분화와 활성화에 중요한 역할을 하며 파치세포가 활성화 되어 치근 표면에 주름 변연을 형성하고 흡수가 진행된다[20]. 계승 영구치가 선천적으로 결손된 유치에서도 치근 흡수가 진행될 수 있다. 치근은 치주인대 세포층에 의해 보호받고 있으며 외상 및 교합력의 증가로 치주인대가 손상 받으면 유치의 치근 흡수가 시작될 수 있다고 하였다[21,22]. 또한 유치의 치주인대는 영구치에 비해 흡수에 감수성이 높고 matrix metalloproteinase 등과 같은 콜라겐 분해효소를 더 많이 생산하여 흡수가 더 빈번하게 일어나게 된다[23].

유치의 생리적 치근 흡수면과 병적 치근 흡수면을 관찰한 연구는 있으나, 원인에 따른 병적 치근 흡수면을 비교 관찰한 연구는 없다. 이 연구에서는 유치 치근 흡수의 원인에 따른 형태학적 차이와 관찰되는 세포를 규명하여 병적 흡수가 진행되는 과정에 대한 이해를 돕기 위해 주사현미경과 광학현미경으로 미세 구조를 관찰하였다.

주사전자현미경 관찰 결과 생리적 치근 흡수면의 흡수와의 크기는 비교적 균일하였으며 뚜렷한 원형과 타원형의 상아 세관이 관찰되었다. 광학 현미경 관찰 결과 생리적 치근 흡수면은 작은 흡수소와들로 구성된 파도모양의 큰 흡수와가 관찰되었다. 균일한 형태의 생리적 치근 흡수면은 유치의 치근 흡수가 전체 치근면에서 느린 속도로 진행되는 과정임을 보여주는 것이다.

외상성 치근 흡수면의 흡수소와는 생리적 치근 흡수면보다 불규칙하고 크며, 특징적으로 편측으로 치근 흡수가 진행된 경우가 많았다. 염증성 치근 흡수면에서 가장 불규칙한 형태의 흡수와가 관찰되었으며 흡수소와의 크기가 상대적으로 크고 다각형이었다. 이는 병적 흡수가 생리적 흡수보다 국소 부위에서 불규칙하고 활발하게 일어나는 과정임을 의미한다. 이러한 결과는 생리적 치근 흡수면보다 염증성 흡수면에서 흡수소와의 크기가 작다고 보고한 박 등[14]의 연구 결과와 다르다. 흡수소와의 형태와 흡수 정도는 교원질 조직의 증식 및 섬유아세포 등 간엽세포의 흡수 정도에 따라 달라질 수 있다고 하였으므로[2], 교원질 조직의 증식과 파괴 정도의 차이에 대한 추가적인 연구가 필요할 것이다.

주사전자현미경 및 광학 현미경 관찰 결과 모든 흡수면에서 흡수소와 내에 다수의 단핵세포가 관찰되었고, 외상성 치근 흡수면에서는 파치세포가 관찰되었다. 3가지 유형의 치근 흡수면 모두에서 파치세포가 많이 관찰되지 않았는데, 치근 흡수 기간 중 휴지기 및 회복 기간에 비해 활발하게 흡수되는 기간이 짧아서 파치세포가 관찰되는 빈도가 적은 것으로 보인다[24].

생리적 치근 흡수면의 흡수소와에서 대식세포 및 섬유아세포가 관찰되었고 이 외의 다른 세포들은 거의 관찰되지 않았는데, 생리적 치근 흡수면에서는 육아조직이 존재하지 않았음을 의미한다. 반면 염증성 치근 흡수면에서는 다핵거대세포, 단핵세포 외에도 간엽세포, 염증세포, 그리고 상대적으로 많은 혈구세포가 관찰되었다. 이는 유치 발거 시 염증조직이 치근면에 부착되어 조직 시편 제작 시 포함되었던 것으로 보인다. 백악아세포, 대식세포 및 섬유아세포 등의 간엽세포는 유치 치근 흡수에 있어서 중요한 역할을 한다[4,12]. 대식세포는 변성된 세포를 제거하며, 섬유아세포는 백악아세포에 의해 유리된 교원질 섬유의 제거와 치근막 섬유의 분해에 주된 역할을 한다[11]. 백악아세포는 교원질 섬유 분해에 관여하고 파치세포의 분화를 촉진하며, 상아질과 백악질의 흡수 및 흡수된 상아질 표면의 remodeling에 관여한다[25]. 이 연구에서 백악아세포를 관찰하기 어려웠는데, 백악아세포는 주로 활성형 파치세포에 인접하여 관찰되며[25] 치근 흡수 기간 중 휴지기 및 회복기간에 비해 활발하게 흡수되는 기간이 짧아 파치세포와 함께 백악아세포도 관찰되지 않았다고 추측해 볼 수 있다.

김과 최[8]는 흡수 진행 정도와 파치세포의 분화 정도에 따라 유치 흡수단계를 흡수 기시기, 파치세포 형성기, 발달기, 성숙기의 네 가지 시기로 구분하였다. 이 연구에서 생리적 치근 흡수면은 주로 단핵세포들만 관찰되므로 파치세포 성숙기의 두 가지 하위 분류인 파치세포와 단핵세포가 혼재된 것과 단핵세포만 존재하는 분류 중 후자에 해당하는 성숙기에 속한다. 외상성 치근 흡수면은 직경 15 - 30 ㎛의 파치세포 및 손가락 모양의 돌기로 덮여 있는 특징적인 소견을 보여 파치세포 형성기로 분류될 수 있다. 염증성 치근 흡수면에서는 단핵세포들의 융합이 왕성하게 일어나고 일련의 흡수와들이 모여 있는 특징이 관찰되어 외상성 치근 흡수면과 같이 파치세포 형성기에 속하는 것으로 보인다. 생리적 치근 흡수면은 외상성 및 염증성 치근 흡수면보다 성숙한 단계의 파치세포가 관찰되어 더 후기 단계이며 느리게 진행되는 과정이나, 외상성 및 염증성 치근 흡수면은 발거 시점에서 덜 분화된 단계이므로 빠르게 진행되는 과정임을 유추할 수 있다.

이 연구에서 생리적 치근 흡수면과 외상성 치근 흡수면에서는 백악질양 조직이 관찰되었으나, 염증성 치근 흡수면에서는 관찰되지 않았다. 염증성 치근 흡수면에서 상아질 흡수를 보상하는 부분적인 백악질 형성이 관찰되었다고 보고한 이전 연구와 다른 결과이다[14].

Sasaki 등[13]은 유치 흡수단계를 흡수 시기에 따라 초기 흡수기, 활동적인 흡수기, 휴지기로 분류하였다. 초기 흡수기에는 활성형의 백악아세포와 파치세포는 상대적으로 거의 관찰되지 않는 반면, 활동적인 흡수기에는 커다란 파치세포가 많이 관찰된다고 하였다. 휴지기에는 상아질 치면이 편평한 백악아세포로 덮여 있고 활성형의 파치세포는 관찰되지 않는다고 하였다[13]. 백악질의 형성은 초기 단계에 주로 관찰되며, 백악질의 침착은 휴지기에 주로 관찰된다고 알려져 있다[1]. Sasaki 등[13]에 따르면 휴지기에 백악질이 대부분 관찰되며 이 시기에 섬유아세포, 백악아세포 및 대식세포와 같은 간엽세포들이 흡수를 주도한다고 하였다.

생리적 치근 흡수면과 외상성 치근 흡수면에서 백약질양 조직의 침착이 관찰된 이유는 흡수의 휴지기에 해당하기 때문이라고 추측할 수 있다. 반면 염증성 치근 흡수면에서 백악질의 형성을 관찰할 수 없었던 것은 병적 조건 하에서 치근 흡수가 급성으로 발생하였으며 휴지기가 지나지 않은 단계에서 발거 되었거나 활동적인 흡수 단계에 속하기 때문인 것으로 추측할 수 있다.

이 연구의 주사현미경관찰에서 단핵세포들로만 구성된 흡수소와가 관찰되었다. 이러한 결과는 단핵세포에 의해 흡수와가 형성될 수 있다고 제시한 김과 최[8]의 의견을 뒷받침한다. 그러나 Vaes[26]는 단핵세포들의 융합에 의해 경조직 흡수 세포들이 형성되며 흡수가 시작된다고 하였으므로 단핵세포 융합 전 단계에서부터 흡수 기능을 수행할 수 있는가에 대한 추가적인 연구가 필요하다.

조직 절편을 광학 현미경으로 관찰하거나 방사선학적으로 연구하는 것은 특정 단면만을 관찰하므로 흡수 단면을 놓치거나 일시적인 흡수 부위를 관찰하지 못할 수 있다[15]. 이러한 점을 보완하기 위해 주사전자현미경으로 관찰하였으며 표면 구조의 3차원적인 형상을 기록하고 표본을 일정 각도까지 기울여서 원하는 부위의 상을 얻을 수 있었다[15].

이 연구에서는 유치의 생리적 흡수면과 병적 흡수면을 광학 현미경과 주사현미경으로 관찰하여 몇 가지 형태학적 차이점 및 세포 조성의 차이를 발견할 수 있었다. 이 연구결과에서 생리적 치근 흡수면은 파치세포 성숙기 및 휴지기에 속하며 외상성 치근 흡수면은 파치세포 형성기 및 휴지기에 속한다. 염증성 치근 흡수면은 파치세포 형성기 및 활동적인 흡수 단계에 속한다고 볼 수 있다[8,13]. 소아에서는 상악 유전치부에 외상이 빈번할 뿐 아니라 우식 이환률 또한 높아서 병적 치근 흡수가 진행되는 양상을 빈번하게 관찰 할 수 있다. 이러한 결과는 다른 원인에 의해 병적 치근 흡수가 진행될 경우 흡수 속도 및 흡수 양상에 대해 예측하여 보호자에게 설명할 수 있는 근거가 될 수 있을 것이라고 생각된다.

이 연구에서 적용한 주사전자현미경 관찰은 세포 수준에서의 판독이 어려우므로 세포를 규명하기 위해서 세포화학적 방법과 투과전자현미경을 이용하는 것이 도움이 될 수 있다[27-29]. 흡수기전에 대한 차이를 규명하기 위해, 투과전자현미경을 통해 초미세구조 및 세포 소기관들을 비교 관찰함으로써 원인에 따른 세포의 기질 변화를 관찰하는 것이 추가적으로 필요할 것으로 생각된다.

Ⅴ. 결 론

이 연구에서는 생리적 치근 흡수와 외상으로 인한 치근 흡수, 치근단 염증으로 인한 염증성 치근흡수로 인해 발거된 상악 유중절치의 치근 흡수면을 주사전자현미경과 광학현미경으로 관찰하여 다음과 같은 결과를 얻었다. 생리적 치근 흡수면의 흡수소와는 원형과 타원형이고 비교적 균일하며 작은 반면, 외상으로 인한 치근 흡수면과 염증성 치근 흡수면의 흡수소와는 다각형이며 크기가 크고 불규칙하였다. 생리적 치근 흡수면과 외상으로 인한 치근 흡수면, 염증성 치근 흡수면에서 다핵거대세포와 단핵세포가 관찰되었으며 염증성 치근 흡수면에서는 간엽세포 및 염증세포가 많이 분포되어 있었다. 생리적 치근 흡수면과 외상으로 인한 치근 흡수면에서는 흡수 표면에 백악질양 조직이 침착되었으나, 염증성 치근 흡수면에서는 관찰되지 않았다.
