Abstract

Chloral hydrate(CH)는 100년이상 동안 진정용 약물로 이용되어 왔다. CH는 1832년 처음 합성되었고, 1861년에는 진정제의 특성이 소개되었으며, 1869년 이후부터는 합성기술의 발달로 이용 빈도가 증가되었다. 1894년 소아에게 CH를 사용하기 시작한 기록이 있다. CH는 최근까지도 안정성에 관한 많은 논란들이 있었음에도 저렴한 가격과 비교적 안전하다는 경험적 이유로 어린이의 치과 진정법에 여전히 사용되고 있다. 그러나, 2006년 미국 FDA의 권고 이후, 2012년부터 미국내 제약회사는 CH를 더 이상 생산하지 않고 있다. 그럼에도 여전히 미국 내 여러 지역에서는 CH 원재료를 이용하여 조제한 현탁액 형태로 진정법에 사용하고 있으며, 2012년 이후에도 사망과 관련한 부작용의 보고는 계속되고 있다. CH는 현재까지 한국에서 소아 진정법에 가장 많이 이용되고 있는 약제이며, 최근 한국에서도 다수의 부작용에 관련한 보고들이 있었다.

최근 진정법을 이용한 소아의 치과치료에, CH를 대신하여 dexmedetomidine, propofol 등 여러 약물들이 소개되었다. 다만 이들을 소아치과 영역에서 사용하는데 있어서는 여러 가지 제한점이 있으며 고려해 볼 사항들도 많다.

이에 CH와 관련된 합병증들과 한국에서의 사용 실태, 그리고 CH를 대체할 수 있는 약물에 대해 알아보고자 한다.

Trans Abstract

Chloral hydrate (CH) has been used in sedation for over 100 years. CH was first synthesized in 1832, the sedative properties were observed in 1861. Because of its easy synthesis, its use was widespread since 1869. There is a record of the use of CH in children as early as 1894. Recently there have been many controversies about safety of CH. Because of the low cost and relatively safe experience CH has still been used for dental sedation in children. After the US FDA recommendation in 2006, US pharmaceutical companies no longer produce commercial CH. However, CH has been used in the form of suspensions prepared from raw materials in many areas of the United States, and reports of adverse events related to death have continued.

CH is the most commonly used drug for sedation in Korea, and there have been some reports of side effects.

Dexmedetomidine, propofol and midazolam were introduced as an alternative for CH. There are various limitations in using them in the pediatric dentistry area and there are many things to consider.

The purpose of this review is to analyze the complications of CH and status of use in Korea, and to introduce alternatives to CH.

Ⅰ. 서 론

어린이의 치과치료는 두려움과 공포를 조절하는 것부터 시작된다. 진정법은 여러 이유로 협조가 어려운 어린이에게 고품질의 치과치료를 제공하기 위해 오랜 기간 이용되어 왔다. 진정법에 사용되는 여러 약물들 중에서 chloral hydrate(CH)는 대표적인 약물이다. 현재CH는 안정성에 관한 논란들이 있었음에도 가격이 저렴하고 사용이 용이하여 어린이의 치과 진정법에 널리 사용되고 있다.

최근에 진정법을 이용한 어린이의 치과치료에 CH를 대신하는 여러 약물들, dexmedetomidine, propofol 등이 소개되었다. 다만 이들을 소아치과 영역에서 사용하는데 있어서는 여러 가지 제한점이 있으며 고려해 볼 사항들도 많다.

이에 CH와 관련된 합병증들과 한국에서의 사용 실태를 분석하고 CH를 대체할 수 있는 대안을 소개하고자 한다.

Ⅱ. 본 론

1. Chloral hydrate

1) History of chloral hydrate

CH는 1832년 처음 합성되었으며[1], 1861년 CH의 진정제 특성이 발견되었고, 1869년 상세히 기술된 이래, 합성이 용이하여 널리 알려졌으며, 임상에서도 자주 사용하게 되었다[2]. 1894년 초에 소아에게 CH가 사용되었다는 기록이 있다[3].

CH는 체내에서 대사되어 trichloroethanol로 변환되며 이 물질이 중추신경계를 억제하여 진정 효과를 나타낸다[4].

CH는 소아에게 진정을 위한 목적으로 투여할 경우 여러 문헌에서 추천하는 용량을 Table 1에 정리하였다.

CH는 그 동안 안정성에 관한 많은 논란들이 있었으나 저렴한 가격과 비교적 안전하다는 경험적 이유로 어린이의 치과 진정치료에 사용되어 왔다. 2006년 미국 FDA는 CH의 안전성 확인을 기존 약물과 같은 수준으로 조사할 것을 권고하였다[5]. 이후, 2012년 미국 내 제약회사는 시럽이나 캡슐 형태의 판매용 CH를 더 이상 생산하지 않고 있다.

2) Adverse event of chloral hydrate

CH의 가장 널리 알려진 부작용으로는 위장관 자극으로 인한 오심, 구토이며, 과다복용 시 운동실조, 혼수, 심 부정맥, 호흡 억제 등이 나타날 수 있다[6]. CH는 권장용량에서는 심혈관계의 영향이 적지만, 과다복용 시 심장 독성을 보일 수 있고[7,8], 심부정맥으로 인한 사망한 사례가 보고되었다[9].

CH의 호흡기에 대한 영향은 투여 용량이 증가함에 따라 커질 수 있다[10]. 신생아 집중치료실에서 CH를 이용한 진정법 하에 다양한 검사를 시행한 보고에 의하면 41.3%에서 부작용이 나타났으며, 산소 포화도 감소가 18.8%, 무호흡 증가가 17.5%를 보였다[11].

미국에서 2012년 사업상의 이유로 제약회사가 판매용 CH 제품의 생산을 중단하였지만, 미국 약전 협약 모노그래프(U.S. Pharmacopeial Convention monograph)에 따라, 약사는 원재료를 제약공급회사로부터 공급받아 연방 식품, 의약품 및 화장품(FD & C) 법의 503A(개별 처방)에 의거 CH를 조제할 수 있다[12]. 또한 파우더 또는 크리스탈 형태의 CH를 현탁액 형태로 조제하여 소아 진정법에 사용하고 있다. 그러나, 조제된 약물은 진정 작용시간이 시판 제품보다 짧아서 이차 약물의 병용 투여가 필요하고 이는 부작용 가능성이 커진다[13]. 2012년 이후에도 미국에서 CH를 사용한 진정법 관련 부작용이 보고되고 있다. 2014년 CH의 과다용량 사용으로 인한 3가지 사고 사례가 보고되었으며, 이 중 두 사례에서는 응급실로 옮겨져 처치 후 귀가하였으나, 한 사례에서는 사망에 이르렀다[9].

CH는 현재까지 한국에서 소아 진정법에 가장 많이 이용되고 있는 약제이며, 2010년 이후 진정법과 관련된 여러 사망 사고가 언론을 통해 보고되었다. 2009년 6월부터 2014년 6월까지 Korean Society of Anesthesiologists database of anesthesiarelated medical disputes에 등록된 진정법 또는 전신마취와 연관된 심각한 부작용의 보고내용을 토대로 105건의 사고가 분석되었다[14]. 이 연구에서 105건의 사고 중 5건은 치과에서 발생한 것이었다. 그리고 105건중 39건은 진정법과 관련된 사고였으며, 사용한 약물은 대부분 propofol이었으나 2건의 midazolam과 2건의 CH가 포함되었다[14].

진정법과 관련된 생명을 위협하는 부작용들은 과도한 약물의 사용과 특히 3가지 이상의 약물을 혼합 사용한 경우와의 상관관계가 높았다[10].

미국 소아치과학회에서 1991년부터 장기간의 연구로 Project USAP: The use of sedative agents in pediatric dentistry를 진행하였다. 가장 최근 발표한 자료는 2010년의 자료이며, 1061명의 설문답변에서 6명이 호흡곤란의 부작용을 경험하였다고 답하였다[15]. 2014년 한국 소아치과학회에서 주관한 연구에 의하면 참여한 111명의 소아치과의사 중 51%가 진정법 중 호흡곤란의 부작용을 경험했다고 하였다[16].

CH와 그 대사산물인 trichloroethanol, trichloroacetic acid는 모두 발암 물질(carcinogen)인 trichloroethylene의 대사산물(metabolites)이다. 아직까지 사람의 치료 용량에서는 암을 유발하지는 않았지만, 동물 연구에서는 암을 유발하였다[17].

3) Status of chloral hydrate in Korea

CH는 미국과 영국에서는 더 이상 생산되지 않는다[18]. 한국에서는 시럽형태(Pocral®, Hanlim)로 한림제약에서 생산하고 있다. 그 동안 10 g/100 mL 용량으로 생산하고 있었으나, 2018년 5월부터 마약류통합관리시스템이 시행됨에 따라 1 g/10 mL의 소포장 단위로 바뀌어 생산된다. 또한 바뀐 법령에 따라 사용하고 남은 CH를 폐기할 때 기존의 향정신성의약품과 같은 절차를 따르도록 하였다. Fig. 1은 남은 CH의 폐기 절차를 정리하였다.

4) Recommendation of dose of chloral hydrate for conscious sedation

여러 연구에서 진정법을 위해 사용하고 있는 약물의 용량을 조사하였다. 2015년 8월까지 한국에서 발간된 논문을 대상으로 진정법과 연관된 검색어를 입력하여 한국의 5개 기관에 수록된 자료를 조사한 연구에서 20개의 논문이 선정되었다[19]. 이들 논문에서 사용된 진정약물들의 용량은 Table 2와 같다.

2015년 Stephen[20]은 진정치료가 필요한 아이들의 협조도와 진료시간에 따른 추천 진정법과 각각의 약물 용량을 제안하였다(Table 3).

2. Sedative alternatives

CH는 쉬운 투여경로와 저렴한 비용 그리고 오랜 사용 경험 등으로 관습처럼 이용되고 있다. 하지만, 환자의 안전과 약물의 장기적인 공급 안정성을 고려한다면 대체재를 찾아야 한다.

1) Dexmedetomidine

Dexmedetomidine은 1999년 미국 FDA의 사용승인을 받고, 2010년 한국에서 식품의약품안전처의 사용 승인을 받았다. 이 약은 알파2 아드레날린 수용체에 작용하여 진정 및 진통효과를 모두 가지고 있으며, 호흡억제가 거의 없어 진정법에 사용할 수 있는 좋은 대안이다[21]. 또한, 전신마취 환자의 술 후 emergence delirium을 줄이는데 술 전 dexmedetomidine의 사용이 도움이 된다는 연구가 있었다[22]. 2017년에는 소아의 치과치료를 위한 진정법에서 dexmedetomidine을 비강내(intranasal) 경로로 사용한 결과 92%의 성공률을 보였고 다른 진정법에 비해 조금 더 길게 잠든 결과를 보고했다[23]. 하지만, 한국에서는 이 약제의 사용 범위가 응급실에 국한되어 있기 때문에 소아의 치과 치료를 위한 진정법에 적용하는 데에는 어려움이 있다.

2) Propofol

Propofol은 빠른 작용시간과 빠른 회복을 보이는 진정, 수면제이다[24]. 그러나 이 약제는 정주(intravenous)의 경로로 사용되고, 깊은 진정 또는 얕은 전신마취 정도의 진정이 유도되므로 치과의사가 단독으로 사용하는 것은 추천하지 않는다[25]. 또한, 국내에서는 3세이하의 경우 사용을 제한한다.

3) Midazolam

Midazolam은 benzodiazepine계 약물이며 최소진정을 목표로 하는 진정법에서 가장 많이 사용되고 있다[26,27]. 약물의 투여방법도 경구, 근주, 정주, 그리고 비강내 등 다양하며 진정수준의 조절도 가능하다. 길항제인 flumazenil이 존재하는 것 또한 장점이다. 다만, propofol에 비해 회복시간이 길어 술 후 감시기간이 길어진다. 미국과 영국에서는 경구로 투여할 수 있는 midazolam이 시판되고 있다[28]. 하지만 국내에서는 아직 식품의약품안전처의 사용 승인된 경구용 midazolam이 없다[29].

Ⅲ. 결 론

소아의 치과치료는 불안과 공포를 줄이기 위한 노력이 병행되어야 하며, 약물을 이용한 진정법은 훌륭한 치료법이 될 수 있다. 그중 CH는 오랜 기간동안 치과의사들이 관성처럼 이용하여 온 약제이다. 그러나, 최근에는 안전에 관한 이슈가 크게 부각되고 있다. CH는 길항제가 존재하지 않으며, 경구로 투여하므로 진정의 깊이를 적정할 수 없다. 또한 오심과 구토를 줄이려는 목적으로 다른 약제와 병행하는 경우가 대부분이다. 이는 약물 간의 상호작용으로 원하지 않는 진정 깊이를 유도하거나 호흡곤란 등의 부작용을 일으킨다.

CH를 사용하는 진정법에서 중등도 진정을 유지하는 것은 매우 어려운 일이다. 따라서, 환자의 안전과 진정 깊이의 조절, 길항제의 존재 여부 등을 고려할 때, CH를 대체할 다른 약제의 사용을 고려할 필요가 있다.
